Compensation under the safe pregnancy (Pour une maternité sans danger) program

Responsibility : Commission de la santé et de la sécurité du travail (CSST)

To have your address changed, you can :

• Complete the on-line form
• Phone, write or go to your regional branch of the CSST

Important
If you have more than one CSST record, say so when you call so that your address can be changed in your other records as well.

Note

If you received compensation during part of the year and you’ve since moved, you must notify the CSST of your new address in order to obtain the RL-5 slip to be attached to your income tax return.
