By telephone
You must provide the following information:
- The exact spelling of the name on the bond (including any initials)
- The Customer ID or the complete serial number of the bond or the par value and series
- The former address
- The new address
In writing
Complete, sign and mail the printable version of the on-line form.
Payroll savings plan holders
Complete the appropriate form, or phone.
